Valiant Living’s Application for Services Process

Valiant Living’s goal is to always provide the best assistance for people who will benefit from our services. To ensure we can offer you or your loved one all the services desired, we utilize the following four-step process. Please remember that we are always here to help.   1. Contact us for an application, release forms, and any other information gathering tools. Alternatively, download the Statewide Request for Services form.
  2. Applications are reviewed by our Admissions Committee Chairperson. She will contact you to in order to gain additional information if needed and to set up a personal visit. That visit can include a tour of Valiant Living’s service settings, both residential and day program. We will invite you and the people who are important to you to come visit. We want to learn more about you and tell you about our agency. This will include going over our consumer handbook, your rights, and our agency policies. In addition, we will also make sure you are aware of all available options.
  3. It is important that all applicants have a Conflict Free Case Manager (CFCM). If the applicant has a CFCM, our Admissions Committee Chairperson will facilitate contact with the person’s CFCM regarding the person’s application for services.  If the applicant does not have a CFCM, our Admissions Committee Chairperson would be happy to help with obtaining one.
  4. Your application will be reviewed by our Admissions Committee. We want to make sure we can provide every service that will fit your personal needs. Regardless of the determination made by the Admissions Committee, our Admissions Committee Chairperson will maintain contact with your CFCM regarding any related recommendations and on-going planning.
